Welcome to my practice.  Below you will see links to the client paperwork for you to fill out and bring to our first session. Please read through the HIPAA Psychotherapist-Client Agreement and print out, sign, and bring in only the last page of this form.  Please also fill out, print out, and bring the Client Information Form, Cancellation Policy, and Credit Card Payment Consent Form to our first session.  The HIPAA Notice of Privacy Practices is just for your records and information and you do not need to bring this to our session.  The Group Guidelines Form and Group Liability Waiver only need to be filled out by clients that are joining Transformations: A Yoga and Mindfulness Psychotherapy Group.  Please let me know if you have questions about any of the paperwork before or during our first appointment.  I look forward to seeing you soon!
